1. Contact your primary care provider with the VA & tell them you need authorization to receive your DOT PHYSICAL EXAM.
*In order to perform the DOT exam the Dr. MUST be certified with the National registry of Certied Medical Examiners (NRCME). Very few VA Dr.’s will have this certification and without it your exam is no good.

3.Call your VA primary care provider with as much advance notice as possible and request authorization to receive your DOT exam at our clinic. I am including a request form to make this easy so all you have to fill in is your name and/or VA ID. You can come by the clinic to pick one up or we can email it to you.
Once you receive your authorization you can come in for your DOT exam and the bill will be sent to the VA. You will not be charged for the exam, the VA will pay for it.
